Required Documents for Registration

ALL of the following documents must be submitted for a student’s registration to be considered complete.

  1. Please print out and bring in the online enrollment form.

  2. Original document of Parent or Guardian/Caregiver picture identification. NOTE: If student lives full time with a caregiver instead of a parent, caregiver must provide a completed Caregiver Affidavit.

  3. Original document of Student Identification: birth certificate, non-expired passport, baptismal certificate, OR a statement by the local registrar or county recorder certifying the date of birth.

  4. Residency document, Primary Proof: One (1) of the following documents with the parent/guardian’s name and address:

    1. Parents/Guardians who are homeowners: Current property tax bill or payment receipt (showing owner name and address). For recent purchases: will accept current closing statement or deed.

    2. Parents/Guardians who are renters: Current Lease Agreement with Manager/Landlord’s contact information and signatures. All pages of the original lease, including signature pages, are needed in addition to any additional documents that extend the lease end date. If the lease end date has expired, or the lease is month-to-month, an additional letter is needed from the manager/landlord indicating the parent(s)/guardian(s) are current residents, which must be dated within the last 30 days.

    3. Shared living arrangements: Must complete BOTH (2) of the following:

      1. Provide a notarized Shared Residency Affidavit, AND

      2. Grant Deed, property tax bill, current rental or lease agreement of the person with whom you are living.

5. Residency documents, ADDITIONAL proof: Two (2) of the following original, and most current, documents with the parent/guardian’s name and address:

  • Utility bill, statement, or payment receipt: PG&E, water, cable, landline phone, or garbage are examples of residency documents.

6. Immunization record (polio, DTP, measles, mumps, rubella, Hepatitis B, varicella). TDaP is required for 7th and 8th graders. TB is optional. See Immunization Requirements.

  • All immunization records must be translated to English by your health care provider PRIOR to your enrollment appointment. You will not be able to complete the enrollment process until immunization records are translated.

7. If your student is entering 1st grade, the health exam form must be completed by your student's physician.
